Proof of immunization compliance is required from听all current and incoming students. - What immunizations are required by 杏吧传媒?
TWO(2) doses of measles vaccine; at least听one听(1) dose each of rubella and mumps vaccine;听ONE听(1) tetanus-diphtheria booster (AT LEAST 10 YEARS CURRENT);听罢奥翱听(2) doses of meningococcal meningitis vaccine OR one (1) dose, if the first dose was given on or after age 16.

Students can request a waiver of the required immunizations by completing the Online Exemption form.听 *If you are听under听18 years of age, a parent or legal guardian’s signature is required on the back/second page of the听Proof of Immunization Compliance form. - How can I submit my immunization record/form?
You can submit your immunization records by filling out this form.听 Immunizations can also be sent to UHS by听mail (P.O. Box 2054, Thibodaux, LA 70310), fax (985-493-2601), or e-mail (healthservices@nicholls.edu).听 You can also upload your immunization records to the听. - How can I receive a copy of my immunization records?
If you wish to obtain a copy of your immunization records, please send an email to听healthservices@nicholls.edu OR complete this request. 听In the email/request, state your name, student ID#, and tell us what you are requesting. 听A copy of your complete immunization record can also usually be obtained from your local parish health unit or your physician鈥檚 office.
