  1. Where can I find the immunization compliance form?<\/strong>
    <\/strong>The Proof of Immunization Compliance form is located\u00a0here<\/a>.<\/a> \u00a0You can also send in a copy of your immunization records.<\/li>
  2. Who should submit proof of immunization compliance?<\/strong>
    <\/strong>Proof of immunization compliance is required from\u00a0all current and incoming s<\/i>tudents<\/em>.<\/li>
  3. What immunizations are required by ÐÓ°É´«Ã½?
    <\/strong>TWO<\/strong>(2) doses of measles vaccine; at least\u00a0one<\/strong>\u00a0(1) dose each of rubella and mumps vaccine;\u00a0ONE<\/strong>\u00a0(1) tetanus-diphtheria booster (AT LEAST 10 YEARS CURRENT);\u00a0TWO\u00a0<\/strong>(2) doses of meningococcal meningitis vaccine OR one (1) dose, if the first dose was given on or after age 16.

  4. Can I sign a waiver for the required immunizations?<\/strong>
    <\/strong>Students can request a waiver of the required immunizations by completing the     Online Exemption form<\/a>.\u00a0 *If you are\u00a0under<\/span>\u00a018 years of age, a parent or legal guardian’s signature is required on the back\/second page of the<\/em>\u00a0Proof of Immunization Compliance form<\/a>.<\/i><\/li>
  5. How can I submit my immunization record\/form?<\/strong>
    <\/strong>You can submit your immunization records by filling out this     form<\/a>.\u00a0 Immunizations can also be sent to UHS by\u00a0mail (P.O. Box 2054, Thibodaux, LA 70310), fax (985-493-2601), or e-mail (healthservices@nicholls.edu<\/a>).\u00a0 You can also upload your immunization records to the\u00a0UHS Patient Portal<\/a>.<\/li>
  6. How can I receive a copy of my immunization records?<\/strong>
    If you wish to obtain a copy of your immunization records, please send an email to\u00a0    healthservices@nicholls.edu<\/a> OR complete this request<\/a>. \u00a0In the email\/request, state your name, student ID#, and tell us what you are requesting. \u00a0A copy of your complete immunization record can also usually be obtained from your local parish health unit or your physician\u2019s office.<\/li><\/ol>
