  • Note: If equipment is not picked up on specified date and time requested, the application will be voided and you will need to start the process over again. Agreement: Student agrees to take full responsibility for any damages or loss of equipment while in the student’s possession. If the camera is lost, stolen or damaged, the student must report it immediately to the appropriate faculty/staff. Student must also repair or replace equipment before the end of the semester. If student does not replace or repair equipment, the student’s grade will be withheld.
